Regulations of the Dokkoon Thai Massage & Spa salon

I. GENERAL PROVISIONS

1. The Dokkoon Thai Massage & Spa massage parlor provides services as part of business activity conducted under the name JOLANTA BUTSARAKHAM A PIECE OF THAILAND with its registered office in Warsaw, ul. Mickiewicza 20/1, 01-517 Warszawa, NIP: 613-156-04-41 and offers services to improve well-being and physical condition. Dokkoon Thai Massage & Spa is not a medical facility within the meaning of the regulations in force in Poland, and its employees do not offer diagnostic services or health services.

2. At Dokoon Thai Massage & Spa, you must comply with these Regulations and follow the instructions of its employees - reception staff and masseuses.

3. The current Price List for the services provided is available in the premises and on the website of Dokkoon Thai Massage & Spa as an appendix to these Regulations.

4. The duration of each service is counted from the moment you enter the massage room until you leave.

5. Purchasing a service from the Dokkoon Thai Massage & Spa offer (including Vouchers) is tantamount to making a statement by the Customer that there are no health contraindications to use the services offered by Dokkoon Thai Massage & Spa and that he/she has read and accepts the terms of these Regulations . Apart from the website, the regulations are also available at the reception.

6. The customer using the services of Dokkoon Thai Massage & Spa is aware that the massages performed in the salon are deep, strong massages with elements of Yoga and after the treatment there may be side effects such as: local inflammation, fever, malaise, muscle pain and many others side effects, especially in the elderly, with contraindications and unsportsmanlike people. With this in mind, the customer has no right to complain in the event of any side effects.

II. CUSTOMER RESPONSIBILITIES

1. During the first visit, the Customer is obliged to read these Regulations and to complete the Customer Survey form by providing true information regarding the state of health and possible contraindications, and to update this data each time in the case of subsequent visits.

2. The client is obliged to immediately inform the person performing the massage about any discomfort or malaise during the procedure.

3. The client is obliged to inform the staff about the improperly performed service during the massage (comments to the quality, wrong treatment, etc.). After the procedure, complaints will not be considered and it will be considered that the massage was performed with due diligence.

III. RESERVATIONS

1. Booking a massage service together with a proposed date of its performance is made by the customer by e-mail, telephone or in person.

2. The date of the massage proposed by the client is binding if it is confirmed by e-mail or telephone by an employee of Dokkoon Thai Massage & Spa. The customer is obliged to cancel or change the date of the service at least 24 hours before the date of the reservation.

3. In the interests of high quality of services, the Customer is obliged to arrive for the booked treatment 10 minutes before the agreed time in order to prepare for the massage. The agreed hour of the massage is the hour of its commencement.

4. If the Client is late for the agreed treatment, Dokkoon Thai Massage & Spa reserves the right to shorten the time of service by the time of delay, and in the case of a delay of more than 15 minutes, to cancel the reservation.

IV. OFFER

1. The current offer is available at the reception and on the Dokkoon Thai Massage & Spa website - www.dokkoon.pl

V. SALE AND REDEMPTION OF VOUCHERS

1. In exchange for the cash equivalent, Dokkoon Thai Massage & Spa issues a Voucher in the form of an invitation packed in an envelope, entitling to perform treatments and massages at the Dokkoon Thai Massage & Spa salon.

2. Vouchers are available for continuous sale and can be for any amount and any treatment from the current offer of Dokkoon Thai Massage & Spa.

3. Vouchers can be purchased at the Dokkoon Thai Massage & Spa salon.

4. At the request of the Buyer, the Voucher can be sent to the address indicated.

5. Fee for sending by post according to the courier's price list.

6. The voucher cannot be exchanged for cash.

7. The voucher is a form of payment for services agreed by the parties.

8. Dokkoon Thai Massage & Spa is not responsible for the destruction, theft or loss of the Voucher.

9. The Buyer and the Voucher holder are not entitled to any claims against Dokkoon Thai Massage & Spa in the event of loss, including theft or destruction of the Voucher.

10. The voucher is valid for 3 months from the date of issue.

11. Dokkoon Thai Massage & Spa does not extend the validity of the Voucher.

12. If the Voucher is not used, the Buyer or the holder is not entitled to a refund.

13. The voucher can be used for another treatment from the offer, of the same value.

14. Persons redeeming the Voucher are treated on the same terms as other clients of Dokkoon Thai Massage & Spa.

15. In order to use the Voucher, it is required to book the date of the visit at Dokkoon Thai Massage & Spa in advance, together with the Voucher number (if it appears on the Voucher).

16. Dokkoon Thai Massage & Spa guarantees the possibility of redeeming the Voucher during the working hours of the Dokkoon Thai Massage & Spa salon within all available dates in the working schedule of the Dokkoon Thai Massage & Spa salon.

17. Cancellation of an appointment related to the use of the Voucher must be made no later than 24 hours before its date by phone, e-mail or in person.

18. The Voucher holder is obliged to present it at the Dokkoon Thai Massage & Spa reception before performing the service.

19. The holder of the Voucher is not entitled to get change if the value of the services is lower than the value of the Voucher.

20. The Voucher holder is obliged to pay the difference in cash or by card if the value of the service provided is higher than the value of the Voucher.

21. Dokkoon Tha Massage & Spa has the right to refuse the service if:
- the validity period of the Voucher has expired,
- damage to the Voucher makes it impossible to read it,
- there are health contraindications on the part of the Voucher holder to perform the procedure.

22. The following information is placed on the Voucher:
- name of the treatment or amount to be used,
- voucher number (there are exceptions),
- date of issue,
- contact information enabling you to arrange a visit as part of the Voucher.

23. Any complaints related to the implementation of Vouchers will be considered by Dokkoon Thai Massage & Spa after submitting a written complaint.

24. The basis for recognizing the complaint is the Voucher.

25. Upon receipt of the Voucher, the buyer or holder of the Voucher declares that he/she has read the Regulations, accepts its content in its entirety and undertakes to comply with its provisions.

26. A specimen of currently valid Vouchers is available at the reception desk and on the website of Dokkoon Thai Massage & Spa.

VI. CONTRAINDICATIONS

1. In most cases, massage is a safe procedure. People who are affected by one of the following situations should not undergo a massage:

- all diseases with high body temperature (above 38° C), acute and subacute inflammation,
- breaking the continuity of the skin, the early period after fractures, sprains and dislocations (in such cases, individual healthy joints can be massaged),
- all kinds of dermatological changes,
- scars, bruises, skin inflammations, varicose veins (in such cases, only healthy parts of the body can be massaged),
- allergies to cosmetics used for massage,
- hemorrhages or a tendency to their occurrence,
- uncompensated heart defects, aneurysms, recent blood clots, recent heart attack,
- malignant and benign tumors,
- post-traumatic haemorrhages in joints and muscles (up to 3 days after the injury),
- inflammation of the veins,- osteomyelitis with fistulas,
- syringomyelia,
- the period of the first trimester and the last month of pregnancy,
- the whole period of pregnancy with complications,
- all cases requiring surgical intervention,
- people after surgery (the massage can be done 12 months after the surgery if it was serious and 6 months after a minor one if the scars are well healed),
- peptic ulcer disease with bleeding,
- liver and kidney stones.

Respiratory system:
- tuberculosis,
- acute inflammation (pneumonia, bronchitis),
- infectious diseases,
- neoplastic changes,
- bronchial asthma during attacks.

Digestive system:
- rupture of gastric ulcer and duodenal ulcer,
- inflammation of the pancreas,
- viral hepatitis,
- cholecystitis,
- inflammation of the bile ducts,
- appendicitis,
- peritonitis,
- parasitic diseases,
- neoplastic changes,
- changes that threaten to break the continuity of the intestines.

The circulatory system:
- state after myocardial infarction (2 weeks),
- advanced atherosclerosis of the coronary vessels,
- fresh inflammation of the valves and heart,
- uncompensated heart defects,
- angina pectoris during an attack,
- hypertension (diastolic 110),
- peripheral vascular diseases,
- vascular diathesis,
- advanced peripheral arteriosclerosis,
- varicose veins with skin lesions,
- varicose veins in the place of occurrence,
- thrombophlebitis (6 months after inflammation),
- Birger's disease III°IV°,
- Reunald's disease III°IV°.

2. Purchasing a service from the offer of Dokkoon Thai Massage & Spa is tantamount to submitting a statement by the Customer that there are no health contraindications to use the services offered by Dokkoon Thai Massage & Spa and that he has read and accepts the terms of these Regulations.

VII. RESPONSIBILITY FOR CUSTOMER'S ITEMS

The customer is responsible for items and valuables left in the Dokkoon Thai Massage & Spa salon.

VIII. RESPONSIBILITY OF DOKKOON THAI MASSAGE & SPA

1. All services provided in the Dokkoon Thai Massage & Spa salon are intended only to improve well-being and physical condition, i.e. they are not health benefits within the meaning of the regulations in force in the territory of the Republic of Poland, and advice provided by masseurs is not medical advice.

2. Dokkoon Thai Massage & Spa reserves the right to withdraw from the service or interrupt the treatment in progress in the event of non-fulfillment or incorrect completion of the Customer Survey form, the Customer's malaise, visible contraindications or inappropriate behavior of the Customer (verbal aggression, behavior with sexual undertones, crossing the boundaries of the staff's sense of security, violation of bodily inviolability, etc.).

3. Minors may use the massage only with the written consent of their parents or legal guardians. Persons under the age of 16 may use the massage only in the presence of a parent or legal guardian.

IX. FINAL PROVISIONS

1. It is forbidden to film or photograph in the area of Dokkoon Thai Massage & Spa without obtaining the prior consent of the person authorized to represent Dokkoon Thai Massage & Spa.

2. Upon receipt of the Voucher, the buyer or holder of the Voucher declares that he has read the content of these Regulations, accepts its content in its entirety and undertakes to comply with its provisions.

3. The Regulations may be changed. Amendments to the Regulations will become effective within 14 days from the date of their placement in the premises and on the website of Dokkoon Thai Massage & Spa.
